HELP! i really cant find where to get the chicago, illinois fingerprint card request form. i need it badly. please help.

You need to request for fingerprint card at this address:

Department of Financial and Professional Regulation

Upon receiving the fingerprint card follow these instructions:

1. Utilize a fingerprint card obtained from this Department/Division
2. Take it to a local police authority in any state to obtain classifiable prints
3. Contact an approved livescan vendor and make arrangements for them to process your fingerprint card. Not all approved livescan vendors process the fingerprint card. Two such vendors that do process the fingerprint card are Accurate Biometrics/Art’s Investigations or Identix. Send the fingerprint card directly to the approved livescan vendor. The fingerprint vendor will advise you of the processing fee.
4. DO NOT send the card directly to the Illinois State Police or Federal Bureau of Investigation (regardless of what is indicated on the card itself).
5. If you follow these instructions, the results of your criminal background check will be electronically forwarded to this Department/Division when available. Failure to follow these instructions may result in a delay in the processing of your application for licensure.

i am confused about my aplication in Illinois, i already sent my requirements and payed the fee for CES application and yet i still i didn’t recieve any letter or confirmation from them.what should i do next?

The payment for the CES Application should be given to CGFNS or the company you chose to evaluate your school records. The CES might take some time before results are sent to Illinois Board of Registered Nursing you can check the status of your CES application online providing you register in their site and provide them with your basic information as well as your CGFNS ID number. Most probably without the CES result your papers won’t be evaluated by the Illinois Board of Nursing yet since it is part of the requirement. it would be best to follow up the status of your CES application first.

Yes that’s not a problem as long as you can provide the basic information for the cardholder such as billing address, name, and other details of the card.

Magandang araw mga kababayan! i have all my documents here in my hands all i need to do is to mail it to California BON, but The Social Security number is bothering me so much, do i need to leave it blank?(since mahigpit na bilin ng Cali BON “NO SS # – NO PROCESSING) or fill it with my ss no.(Philippines)? anu ba talagang ss no ang hinahanap ng BON sa california? SS ng pinas or SS ng america? kung sino man ang makakatulong thanx!

The SSN that they are asking for is the one for the US. Sadly, you cannot get one not unless you are a legal resident/ citizen of the US. You can just place NOT APPLICABLE in the space for the SSN.
