Q&A: Sample Format of Letter of Explanation, Change of Address and more…

hi ms. joan … just want to ask some question ,or anyone out there guys…if ur familiar with this form “REQUEST FOR REAPPLY/REPEAT EXAM”… coz im quite anxious abt filling up this -
LAST EXAM APPLIED FOR: month / year   LAST EXAM TAKEN: month / year

for example I took the exam last oct.10, 2008 is this correct?
LAST EXAM APPLIED FOR: October / 2008   LAST EXAM TAKEN: October  / 2008

pls correct me if im wrong…thanks a lot! ^_^

I am familiar with the form. Please take note that you will just fill out this form if you are to RETAKE the examination. If this is your first time applying to the Board there is a separate form. Last exam applied for means when was the last time you submitted your application. As for last exam taken place the month and year when you last took your NCLEX-RN examination. So it is not possible to have the same date for both.

You can view my sample letter format at the link given. But I suggest together with the letter pass an original copy of certificate of passing from PRC as proof.

Hi. I hope you could help me. Can I be the one to send the requirements needed from the school (such as the TOR, breakdown, etc.) together with my application form (as in all in one envelope)? Because the school gave everything to me including an envelope with the name of my school and a signature. And about the application fee, does that mean that as soon as I have mailed my application form to California, I should also pay the fee using credit card? Thanx a lot in advance.

You CANNOT mail together your application form and school forms. I know a lot of schools in the Philippines do not offer mailing service so what you can do is have all the school requirements place in a sealed envelope and have them sign on the flap of the envelope or place the school seal. Then have it mailed but make it appear that the school is the one mailing the documents. You have to inclued all the fees when you submit your application. I do not think they accept payments via credit card. You can pay them via bank draft.

hi I need help. I am taking the nclex this coming december. My current address is in los angeles but I will be moving to Philadelphia the day after my nclex. I want the results of my test to be mailed to my new address then. How would I inform the board regarding this matter? pls help me. thank you very much.

They have a change of name/address form which you can print by clicking on the link provided. Fill out all the necessary information and inform them that you do not have a license number yet and are just about to take the exam thru a letter.

dear Ms.Joan, I am spreading the word about your site:) Ma’am I have another inquiry. I just received a letter from Cali-BON that upon initial evaluation i need the to send the ff requirements: 

  • 1. Copy of NURSING LICENSE from the Philippines.
  • 2. Copy of NURSING DIPLOMA from the Philippines.
  • 3. TOR
  • 4. RLE
  • 5. Educational Breakdown

      With regards to nos. 3, 4 and 5. My school will be sending them on December 5. My problem is nos. 1 and 2. I sent them a letter of explanation together with an original copy of my Certificate of Board Passing and Certificate of Board Rating. Why are they still asking me for my license/diploma.

      I also read from this site that nursing diploma is the certificate of registration from PRC and not the school diploma. Then what is the nursing license? is it the ID? ma’am thank you again. I know that you will be able to shed light on these things.

      Wow this is a first because usually the certificate of passing suffices. Like what I always say  the evaluation is a case to case basis it just so happened that the evaluator wanted these documents from you. So the only thing you can do is provide it for them. As for item #1 this means they want a copy of your actual PRC license. And for the #2 item they might be pertaining to the certificate PRC is giving when you become a RN or your BSN diploma from your school. To be safe I would suggest giving a copy of both of those diplomas. Also, make sure when submitting these documents you attach a copy of the letter that they sent you so they know what those things are for.

      Q&A: Retaking the NCLEX-RN Exam, Filling Out The Forms and more…

      hello again maaam…. ask ko lang po maam kung what po gagmitin na pangdikit sa picture..is it glue or scotch tape po? ASIAN po ba ang ilalagay ko dun sa race or dapat po may sundin na code.. un pong sa application form na color of eyes.. BLACK po ba ang ilalagay or BLK po gaya sa fingerprint card. and dapat po ba maaam na entire full name ang ilalagay sa signature part both sa application form at fingerprint card?.. yung sa alias(AKA)at SSN maaam will i leave it blank or lagay ko na lang po na NONE or N/A?…help me maam JOAN please…tnk u po

      You need to TAPE the four sides of the picture. It depends on what state you are applying but if you are applying in California you can view their instructions with regards to fingerprinting on page 5 of the application. In terms of the race they did not place any specific codes so you can just write ASIAN. As for the eye color in the application form just write the whole word since the abbreviations were intended only for the fingerprint card. There is no specific rule that was mentioned in terms of what signature you should use. My tip, is just to use a signature that you frequently use and one that is official such as the one you use for bank accounts etc.

      hello ma’am good day! puwede po bang magpa-change ng mailing address after my ATT have already arrived? state of California po at di pa kami nakapagpa-sched ng exam. paano po magparesend ng result ng NCLEX California state if after 2 mos wala pa pong nareceive… thanks po!God bless.

      Can I ask why you need an address change for your ATT? I am afraid that may incur extra fees if you want your address to be changed. You can change your profile information at PearsonVue by logging in to your account and updating yor profile. But that won’t change the information in your ATT. If you are worried that your results will be sent to a wrong address then you have to contact the California Board of Registered Nursing about that. They have this form for change of name and or address you can click on that link provided for the form. Just write a cover letter stating that you are an applicant and haven’t taken the examination yet but your address is changed. If you want to inquire about the status of your results you can give them a call and inform them about your situation. Most of the time they are willing to resend or check the status of your results.

      hello ms. joan.  I failed nclex first attempt, CA BON.  I took it last Oct 10 in the Phils, when can I apply for next exam? or when can I schedule my next exam? Can I take the exam again exactly 45 days after my 1st exam?  Iv’e read abt 45 days waiting period but I don’t understand, if after 45 days I can re-apply or exactly after 45 days I can already schedule for another nclex exam? My state is CA BON, what if I want to change my state to Vermont? What will I do? Pls Ma’am Joan, enlighten me.  I just rcvd my result this afternoon and am disappointed.I hope you can help me.  Thank u so much! More power to you!

      I am very sorry to hear this news. Here is the thing, I just worry that there might be conflict if you apply in Vermont while still having an application in California. What you can do later on is once you pass the NCLEX-RN exam for California and then transfer your license to Vermont. I think that would save you time and money. When you failed the exam, they sent you this form for re-examination. I suggest you to fill that out and pay the corresponding fee as soon as you can. They will reprocess your application and that normally takes 1-2 months and then they will mail you your eligibility notice to be able to take the examination. So by the time you receive your eligibility the 45 day grace period is expired. The 45 days is only for registering in PearsonVue not with the California Board of Nursing.

      You can get through this obstacle. If it helps I will be posting later on an article for possible review materials you can use  that might be able to help you. Also, I will post my article about my NCLEX-RN experience and hopefully that will inspire you to pursuit this dream of yours. Never be discouraged and this stumbling block is there to make you realize how much you want this. Good luck! and check on the site once in a while to see if I have posted the articles.

      Here is the thing the New Jersey Board of Nursing requires you to have a CES Report to evaluate your studies and part of the questions there is about having licensed or not. If you do not have one, just explain the reason why you do not hold one. I do not know what effect that would have with the evaluation of your credentials but nonetheless, just tell the truth and explain your case accordingly. Also, even if you are a US Citizen you took nursing outside of the US and they have different requirements from those who graduated in the US. They might also require you to take the IELTS aside from CES Credential report from CGFNS. You can view the application form for New Jersey Board of Nursing here.

      hi, jaz wanna ask kasi i’ve been here in france for almost 10yrs now.i have practiced my profession as care giver lang ang equivalent coz they don’t accept asian graduates provided kelangan i-evaluate pa.for my 1st and a half ko dito i worked as a private nurse to a pt with parkinson’s dse,yr 1999-early 2001.after that,i stopped kasi i got married.only this time that i realized i have to pursue my career as a nurse.i tried here,pero i have to study 3yrs to complete their requiremnts & i have to improve my french language.when my bro in law and her wife who were dentists went in CA for a conference,they met a friend who is a Fil nurse.i got interested & i tried to search in the net the website & the instructions.ask ko lang,yung fingerprint ba is available in any municipal branches or police stations?will it be requested fr BON a copy of it pra magfingerprint?what advse can u give me,shall i submit the application first,at the same tym d request for the TOR fr my school in PH?kasi i have my 10yrs US Visa w/c i acquired here in Paris.can u give me ideas and advse?thnks so much and God bless u!

      The actual fingerprint card needs to be requested in the CA Board of Registered Nursing. But it can be accomplished usually in any municipalities or police agencies I am not sure though if it would be the same there in France. Processing of application can take 3-6 months. You can submit your application and then at the same time ask request from your school the necessary documents and have them fill out the forms that the CA Board of Registered Nursing is asking them to answer.       Hi Ms. Joan, may gusto lang po ako itanong. I applied NCLEX thru NCLEX Exam Application Center (NEAC), the one located in St. Thomas Square,Morayta. They were also the one who mailed my application via DHL courier last October 21, 2008 and an email was sent to me that my application was safely delivered at ROSEVILLE, CALIFRONIA on October 23, 2008.

      Now my questions are these:

      - The BON address you stated above is in SACRAMENTO, Cali. Then why my application was delivered at Roseville? Is Roseville a part of Sacramento?

      Here is the thing I really do not know how companies such as NEAC work and what their reason for having your documents sent at Roseville, CA. The office of the CA BON is located at Sacramento, CA and Roseville is about 30 miles away from Sacramento. Another thing, is maybe your documents were still in transit on its way to Sacramento. Was the final address destination in Roseville or Sacramento?

      - I haven’t mailed my school credentials yet because my TOR together w/the other forms i submitted to the registrar will be release by November 15 only. What will happen to my application that arrived to Cali, will they start processing it or they will wait for my school credentials before processing anything? Useless lang ba kahit maaga ko napasa yung application form dahil late yung school credentials or start nadin nila i-process yung application ko once they have it tapos they will just wait for my school credentials to arrive? Please enlighten me miss Joan. Thank You so much.

      As far as I know they will not process your papers not unless its paid but the thing is though there really is nothing much to process even with your application forms there. Bulk of the work comes with the school documents that the school sends. We really do not know what is going on or how they process it but the important thing on your part is that you make sure that the documents arrive there safely and all information are correct so that you prevent further delays with your application.

      Verify with them first if they accept credit card payments. If they do, most common information asked when paying via credit card are the following:

       

      • Card Holder’s Name
      • Credit Card Number
      • Type of Card (MasterCard, Visa, JCB, Amex) depends on what they accept
      • Expiration Date
      • Billing Address
      • Security Code (last 3 or 4 digits near the signature area at the back of the card)

      So just be ready with these information when you pay.

      hi ms. joan, just want to ask you if however you pass th board exam in california but you decided to go to another state do they require you to stay in cali to work there or you can just endorse your license to another state?

      Nope you just need to fulfill the requirements in the state where you intend to transfer.

      I recently passed the NCLEX and in the letter that the Cali BORN gave me, it indicated that I need to send them a Social Security Number (SSN) within 3 years in order to be registered and have a license. If I fail to do so, they will “destroy my file”… So NCLEX “does” expire in California, as you have to take it again if you dont provide the BORN with an SSN within 3 years, OR if you dont apply for reciprocity. I hope this little bit of info helps. Please be aware of this if you don’t plan on or have no means of going to the US. Thanks!

      i wrote my e-mail add too slopy thats why it is miss spelled sa application ko sa cabon, so pag check ko ng status ko thats when nalaman ko,. now since mali ung e-mail add ko kelangan ko mag send ng letter informing to change / correction  kaso im having quite dificulty makng a letter talga huhu.. i tried many times calling them pero ganun din busy.. me can u pls help me.. send me a sample letter?.. thnx in advnce.. tc.. godbless

      They won’t change your email address when you call them. They need a written documentation from you informing them about the change or correction in your email address. Just state in your letter that you are  informing them about the correction in your email address. The letter does not need to be long make it short and straight to the point.

      Thank you so much for the kind words. Hearing something like that makes all the effort worth it. 

      By the way, I am  a Registered Nurse in the Philippines and I am currently here in the United States as a tourist, I filed an application for NCLEX-RN just this day, mail it to CA BRN this day also and I hope that you can help me regarding my problem, some say that I might be able to take the exam even without a SSN, but I am afraid that waiting for my Eligibility and ATT will took a long time since my status will expire this March 2009, I arrived here last September.

      You can really take the examination without the SSN. They will just need your SSN later on once you passed the exam for your license. Average application takes 3 months that is if your application does not have any problems whatsoever. One idea I can give you is that after a month of your application reaching the CA BON give them a call and inquire about the status of your application. Please take note that average processing time of application can take about 3-6 months. So while waiting for your eligibility just utilize your time wisely so that by the time it arrives you are prepared to take the exam.

      Sadly, I learned in one of the post that they require a diploma coming from the PRC to my dismay I attached my school diploma, what do you think will be the best action for that, I already sent them my application and other requirements coming from the school, such as request for transcript, breakdown of international grades and nursing program verification…. I wanted to thank you in advance and I will appreciate your feedback on this…GODBLESS YOU MORE!

      Your application will not be forfeited if ever there are any documents they would ask from you they will write you a letter mentioning any deficiencies from your application. 

      Ma’am Joan, I would also like you to know that I just passed the Local Board last June 2008 and I don’t have my license with me yet…also ma’am I already pay for my application when I mailed it since they require it, do you think I can mail my certificate of passing with an attached letter so that my application will not be forfeited?

      You can do that. Just make sure that you explain very well in your cover letter the reason why you are mailing a copy of your certificate of passing.

      tnx maam for your response and for your dedication to help us…I just want to ask again maam kung yung sa signature part po ba dun sa application form at fingerprint card ay dapat po na entire name or full name po ang ilalagay dun.Im bothered about this kasi po shortcut lng ng name ko po ung nilagay ko, as in ung pirma lng na short po. may nagsasabi po kasi maaam na dapat po ay entire name ang ilalagay. accepted po ba ung signature ko na ganun maam or better po na palitan ko na lng po. one thing maam is yung mga requirements na galing po sa skul ay bngay nman po sa akin wid envelop po na sealed.. ako na lang daw po magpadala nun.. pwede ko po bang ipadala un via airmail or fed-ex or better po na regular na lng po? at pwede po ba na ipafed ex ung mailing address nila na PO BOX… or ung Street Address for overnight or in-person delivery na lng po.. kindly answer my quiries maaam…tnx a lot maam…

      What state are ayou applying in? But as far as I know in terms of signature as long as your signatures are consistent and not unless stated that you should sign your complete name then I guess your short signature would be fine. But just take note of the signature you used in the documents as this may be the signature you would be using for your documentation with them. As for the school documents, you can use regular or special couriers that will depend on you since there might be price differences but of course special couriers will arrive faster than regular mail. In addition, make sure that even if its going to be you who will mail the documents make it appear that the school is the one sending it. 

      Most special couriers will not accept PO Box addresses for mailing. I need to know what state you are applying in and most Board of Registered Nursing sites have their physical address written in their website.

      What form or process in the CGFNS are you talking about? I need more information about your application.

      hello maam…kailangan ko po bang ifill up ng buo ung birthplace ko sa fingerfrint card or pwede ko po bang ilagay na lang dun ung province where I am born kasi po hindi na po kasi kakasiya pag buong buo pa po ang ilalagay ko dun..about po sa SSN number..pwede ko po bang ileave blank un or i will just mark it NONE or N/A?…one thing that bothers me maam is about the fold sa fingerfrint card KO PO kasi po medyo nafold ung isang box ng LEFT RING FINGER..OK lng po ba un maam or I will need to change it.. at kailangan po ba na magkapareho ang signature sa fingerprint card maam at sa application form kasi po entire name ko po ang nalagay ko dun sa application form peo po sa fingerprint card  hindi na po.. panu po un maam….maraming salamat po

      Unfortunately, I don’t have a copy of the fingerprint card right now but if it lets you state the birthplace then go for the more specific which is the city where you were born rather than the province. Leave the SSN area blank if its not a major fold or dent then it would be fine. But to be safe, what you can do is to send to the Board two (2) copies of your fingerprint card so that just in case the one gets rejected they have another copy in hand. I would suggest to have same signatures as it may conflict and complicate your application.

      It depends on your school’s policy and protocol. But most of the time that is the case the registrat is the one who fixes the TOR and then the rest of the forms are filled out by the Nursing Department. The important thing is that the school answered the forms and then make sure that you have all the items placed in a sealed envelope with signature and school seal and if possible let the school send the documents if not, make sure that when you mail it make it appear that the school was the one who mailed it. CGFNS are pretty strict when it comes to these requirements.
