hello again maaam…. ask ko lang po maam kung what po gagmitin na pangdikit sa picture..is it glue or scotch tape po? ASIAN po ba ang ilalagay ko dun sa race or dapat po may sundin na code.. un pong sa application form na color of eyes.. BLACK po ba ang ilalagay or BLK po gaya sa fingerprint card. and dapat po ba maaam na entire full name ang ilalagay sa signature part both sa application form at fingerprint card?.. yung sa alias(AKA)at SSN maaam will i leave it blank or lagay ko na lang po na NONE or N/A?…help me maam JOAN please…tnk u po

You need to TAPE the four sides of the picture. It depends on what state you are applying but if you are applying in California you can view their instructions with regards to fingerprinting on page 5 of the application. In terms of the race they did not place any specific codes so you can just write ASIAN. As for the eye color in the application form just write the whole word since the abbreviations were intended only for the fingerprint card. There is no specific rule that was mentioned in terms of what signature you should use. My tip, is just to use a signature that you frequently use and one that is official such as the one you use for bank accounts etc.

hello ma’am good day! puwede po bang magpa-change ng mailing address after my ATT have already arrived? state of California po at di pa kami nakapagpa-sched ng exam. paano po magparesend ng result ng NCLEX California state if after 2 mos wala pa pong nareceive… thanks po!God bless.

Can I ask why you need an address change for your ATT? I am afraid that may incur extra fees if you want your address to be changed. You can change your profile information at PearsonVue by logging in to your account and updating yor profile. But that won’t change the information in your ATT. If you are worried that your results will be sent to a wrong address then you have to contact the California Board of Registered Nursing about that. They have this form for change of name and or address you can click on that link provided for the form. Just write a cover letter stating that you are an applicant and haven’t taken the examination yet but your address is changed. If you want to inquire about the status of your results you can give them a call and inform them about your situation. Most of the time they are willing to resend or check the status of your results.

hello ms. joan.  I failed nclex first attempt, CA BON.  I took it last Oct 10 in the Phils, when can I apply for next exam? or when can I schedule my next exam? Can I take the exam again exactly 45 days after my 1st exam?  Iv’e read abt 45 days waiting period but I don’t understand, if after 45 days I can re-apply or exactly after 45 days I can already schedule for another nclex exam? My state is CA BON, what if I want to change my state to Vermont? What will I do? Pls Ma’am Joan, enlighten me.  I just rcvd my result this afternoon and am disappointed.I hope you can help me.  Thank u so much! More power to you!

I am very sorry to hear this news. Here is the thing, I just worry that there might be conflict if you apply in Vermont while still having an application in California. What you can do later on is once you pass the NCLEX-RN exam for California and then transfer your license to Vermont. I think that would save you time and money. When you failed the exam, they sent you this form for re-examination. I suggest you to fill that out and pay the corresponding fee as soon as you can. They will reprocess your application and that normally takes 1-2 months and then they will mail you your eligibility notice to be able to take the examination. So by the time you receive your eligibility the 45 day grace period is expired. The 45 days is only for registering in PearsonVue not with the California Board of Nursing.

You can get through this obstacle. If it helps I will be posting later on an article for possible review materials you can use  that might be able to help you. Also, I will post my article about my NCLEX-RN experience and hopefully that will inspire you to pursuit this dream of yours. Never be discouraged and this stumbling block is there to make you realize how much you want this. Good luck! and check on the site once in a while to see if I have posted the articles.

Here is the thing the New Jersey Board of Nursing requires you to have a CES Report to evaluate your studies and part of the questions there is about having licensed or not. If you do not have one, just explain the reason why you do not hold one. I do not know what effect that would have with the evaluation of your credentials but nonetheless, just tell the truth and explain your case accordingly. Also, even if you are a US Citizen you took nursing outside of the US and they have different requirements from those who graduated in the US. They might also require you to take the IELTS aside from CES Credential report from CGFNS. You can view the application form for New Jersey Board of Nursing here.

hi, jaz wanna ask kasi i’ve been here in france for almost 10yrs now.i have practiced my profession as care giver lang ang equivalent coz they don’t accept asian graduates provided kelangan i-evaluate pa.for my 1st and a half ko dito i worked as a private nurse to a pt with parkinson’s dse,yr 1999-early 2001.after that,i stopped kasi i got married.only this time that i realized i have to pursue my career as a nurse.i tried here,pero i have to study 3yrs to complete their requiremnts & i have to improve my french language.when my bro in law and her wife who were dentists went in CA for a conference,they met a friend who is a Fil nurse.i got interested & i tried to search in the net the website & the instructions.ask ko lang,yung fingerprint ba is available in any municipal branches or police stations?will it be requested fr BON a copy of it pra magfingerprint?what advse can u give me,shall i submit the application first,at the same tym d request for the TOR fr my school in PH?kasi i have my 10yrs US Visa w/c i acquired here in Paris.can u give me ideas and advse?thnks so much and God bless u!

The actual fingerprint card needs to be requested in the CA Board of Registered Nursing. But it can be accomplished usually in any municipalities or police agencies I am not sure though if it would be the same there in France. Processing of application can take 3-6 months. You can submit your application and then at the same time ask request from your school the necessary documents and have them fill out the forms that the CA Board of Registered Nursing is asking them to answer. Hi Ms. Joan, may gusto lang po ako itanong. I applied NCLEX thru NCLEX Exam Application Center (NEAC), the one located in St. Thomas Square,Morayta. They were also the one who mailed my application via DHL courier last October 21, 2008 and an email was sent to me that my application was safely delivered at ROSEVILLE, CALIFRONIA on October 23, 2008.

Now my questions are these:

- The BON address you stated above is in SACRAMENTO, Cali. Then why my application was delivered at Roseville? Is Roseville a part of Sacramento?

Here is the thing I really do not know how companies such as NEAC work and what their reason for having your documents sent at Roseville, CA. The office of the CA BON is located at Sacramento, CA and Roseville is about 30 miles away from Sacramento. Another thing, is maybe your documents were still in transit on its way to Sacramento. Was the final address destination in Roseville or Sacramento?

- I haven’t mailed my school credentials yet because my TOR together w/the other forms i submitted to the registrar will be release by November 15 only. What will happen to my application that arrived to Cali, will they start processing it or they will wait for my school credentials before processing anything? Useless lang ba kahit maaga ko napasa yung application form dahil late yung school credentials or start nadin nila i-process yung application ko once they have it tapos they will just wait for my school credentials to arrive? Please enlighten me miss Joan. Thank You so much.

As far as I know they will not process your papers not unless its paid but the thing is though there really is nothing much to process even with your application forms there. Bulk of the work comes with the school documents that the school sends. We really do not know what is going on or how they process it but the important thing on your part is that you make sure that the documents arrive there safely and all information are correct so that you prevent further delays with your application.

Verify with them first if they accept credit card payments. If they do, most common information asked when paying via credit card are the following:

 

  • Card Holder’s Name
  • Credit Card Number
  • Type of Card (MasterCard, Visa, JCB, Amex) depends on what they accept
  • Expiration Date
  • Billing Address
  • Security Code (last 3 or 4 digits near the signature area at the back of the card)

So just be ready with these information when you pay.

hi ms. joan, just want to ask you if however you pass th board exam in california but you decided to go to another state do they require you to stay in cali to work there or you can just endorse your license to another state?

Nope you just need to fulfill the requirements in the state where you intend to transfer.

I recently passed the NCLEX and in the letter that the Cali BORN gave me, it indicated that I need to send them a Social Security Number (SSN) within 3 years in order to be registered and have a license. If I fail to do so, they will “destroy my file”… So NCLEX “does” expire in California, as you have to take it again if you dont provide the BORN with an SSN within 3 years, OR if you dont apply for reciprocity. I hope this little bit of info helps. Please be aware of this if you don’t plan on or have no means of going to the US. Thanks!

i wrote my e-mail add too slopy thats why it is miss spelled sa application ko sa cabon, so pag check ko ng status ko thats when nalaman ko,. now since mali ung e-mail add ko kelangan ko mag send ng letter informing to change / correction  kaso im having quite dificulty makng a letter talga huhu.. i tried many times calling them pero ganun din busy.. me can u pls help me.. send me a sample letter?.. thnx in advnce.. tc.. godbless

They won’t change your email address when you call them. They need a written documentation from you informing them about the change or correction in your email address. Just state in your letter that you are  informing them about the correction in your email address. The letter does not need to be long make it short and straight to the point.

Hello Miss Joan!Its me again twinkle!I mailed po ung application ko kasama ung requirements at forms galing sa school ko last june after ng Local board exam ko.3 wks later nka recieved me ng letter from them sabi nila after daw nila nireviewed ung application ko kulang daw ako ng $19 for fingerprint fee.Na recieved na po nila ung additional payment when I called them pero ung application ko daw po di pa nila na process hintay daw po ako ng 24 wks.Di po kaya maging problema ung $19 sa pagprocess ng application ko.Tinanong ko din po sila kung ano pang kulang sa mga requirements sabi nung nkasagot di nya raw alam. Worried po ako kc baka itanung nila ung license ko kahit my explanation letter me.By the way,naipasa ko na ung local board ko last june exam pero december pa ung releasing ng license ko..

First of all how much was the money that you sent together with your application? For fingerprint processing and application fee the total would be $126. The thing is they informed you of the $19 that was lacking and you complied with it and they verified that they received the amount. Average processing of eligibility is 3-6 months depending on the bulk of applicants and other factors. Here is what I can suggest you can follow up your application after a month or two just so you know where you stand. If there are lacking requirements with your application they would inform you via a letter just like what they did when they said your application lacks $19.

I am very puzzled with the NCLEX procedures. i did read lots of information regarding of NCLEX such as application, fees, etc. yet i am still confused. i do have lots of questions in my mind. Would you mind? I am planning to take NCLEX. So i choose CALIFORNIA as my state of option.
Not at all. I mean this is what the site is for to help out people with their application so lets get started with your queries.
1. How much  is really the application fees? Some would say 107, 126 or 200? Which is which?
I know this can get confusing the prices are different depending on what services you are going to use from the California Board of Registered Nursing. So let me give you the breakdown of prices to explain which prices are for what.
  • Application Fee $75
  • Interim Permit $30
  • Fingerprint Process $51
Obviously, you will need to pay the $75 application fee to process your application. In addition, you also have to avail of the fingerprint process fee of $51. All in all total is $126 if you are interested in getting an interim permit then you just add $30 to the total. 
2. Do i have to leave blank in SSN in the application form since i don’t have any.?
Yes just leave it blank.
3. Can u breakdown the list of things before passing my application to CA BON for the first time?

4. do i have to send the demand draft together with the application for the first time? or do i have to wait my eligibility then thats the time i would pay?
You have to send it together with your application or else it will not be processed.

Here are the guidelines taken from the California Board of Registered Nursing website:
Applicants must complete all items which are marked by a black “X” on the card. To facilitate prompt and accurate processing of the fingerprint card by the DOJ and FBI, type or print legibly in BLACK INK all requested information on the card. If any color other than black is used, the card will be rejected and another card will have to be completed and submitted.
Use the abbreviations listed below for the physical description items:
• Height (HGT) – Express in feet and inches. Do not use fractions of an inch; round off to the nearest inch. DO NOT USE THE METRIC SYSTEM. Correct example: 5′ 9″.
• Weight (WGT) – Express in pounds. Do not use fractions of a pound; round off to the nearest pound. DO NOT USE THE METRIC SYSTEM. Correct example: 139 lbs.
• Color of EYES – Black BLK Gray GRY Blue BLU Green GRN Brown BRN Hazel HZL
• Color of HAIR – Bald BAL Gray GRY Black BLK Red/ Auburn RED Blonde BLN Sandy SDY Brown BRN White WHI
Each applicant MUST have his/her fingerprints imprinted only in BLACK INK on fingerprint card. Fingerprints should be taken at a local law enforcement agency. There may be a fee for this service. We advise that you should call first as to a convenient time.
DO NOT FOLD FINGERPRINT CARD. Use a 9″ X 12″ envelope to return your completed application and fingerprint card with fees. Write “DO NOT FOLD” on the envelope. If your card is folded, you will need to complete and submit a new fingerprint card. THIS WILL CAUSE A DELAY IN DETERMINING YOUR ELIGIBILITY FOR EXAMINATION OR LICENSURE.

Q&A: Application Fees In California, Attending Two Nursing Schools, Copy of License and more…

hi, gud day. i would just like to ask what do you think i would right in the application form concerning my professional education. i am a transferee, and attended 2 nursing schools. would i just write the school from where i graduated? and what date would i write in the entrance date? the date i started the course in my 1ST school, or the entrance date in my 2nd school where i officially graduated? i’m confused on what to write and just want to make sure that i would not commit any errors in my application..

i would also like to ask if its ok that i requested in the 2 schools i attended for the breakdown of education program, verification, and request for transcript? when i received my documents in the school that i graduated, the information about my anatomy and physiology subject was blank since i took it at my first school. i was then advised to go to my first school and request it there. is that ok that 2 schools will send my documents?

i’m applying for nclex-rn in california by the way. i hope that you could help me and advice what you think is best for me to do. thank you very much.

With regards to your first question you need to declare both schools where you studied nursing related subjects especially when some of your nursing subjects are left blank since you completed it at the other school. With that in mind, you also need to submit the school forms to the other school or else your application might be incomplete. Write the entrance date in both schools and indicate clearly which entrace dates are from what schools.

hello,,, I just read a post that there are some changes in requirements for the NCLEX CA. Do I really need to submit a photocopy of my license? What if wala pa kong license, can I submit an expalanation letter with the attached copy of my certificate of passing of NLE? Nagrequest na po ako sa school but then di ko pa namamail yung requirements k… may problem po b if nauna magsubmit yung school sa BON before my requirements? thank you..

If you do not have your license yet a certification of passing from PRC with an attached letter of explanation may suffice. If the board then asks you for a copy of your license later on then you can provide them with that at a later time. There is no problem if your school requirements arrive ahead than your personal application. Though I don’t want to risk your school forms getting misplaced so send your personal application as soon as you can.

It depends on what you want to avail so here is a breakdown of the fees: 

  • Application Fee $75.00
  • Fingerprint Card Processing $51.00
All in all total is $126. The interim permit is optional if you intend on getting one you pay an additional $30. You send the fees together with your application or else it will not be processed.
