Thank you for answering my question. Gusto ko lang po iclarify, gagawa pa po ba ako ng letter to endorse yung bank draft sa BON?

I just think that since it comes together with your forms I do not think it would be necessary. But just in case, if you feel like you can be questioned you may place a note and state your reason. Its up to you which one to do.

hello! i am applying for CA BON.. my questions are:since i dont have a bank account, is it ok if i use my father’s account for the bank draft and his name appears on the check? and he has to sign at the back of the check for endorsement? the fingerprint card is asking for the ‘race’ and the code is not included in the instructions, unlike in hair and eye color where options are available. i tried typing ‘asian’ but the space is not enough. should i type ‘a’? thanks in advance! god bless!

That is fine since the bank draft will be attached with your application. As for the race I think it would be better to write the whole word (ASIAN) rather than creating your own abbreviation since like what you said there were no instructions whatsoever. It might be too vague to just put “A” as your race as it can mean a lot of things.  Hope this helped thanks!

I would like to share my experience to those who have this problem with CA SSN..My result will expire this Nov..2009 and thankfully to this blog from Pinoy RN info.. I was completely relieved because who might think that there is no hope with this matter… I am also a Pinoy RN who is frustrated with this retrogression and of course I’m stil waiting since my PD is Feb 2007 and since the cut of date for Philippines is May 2005 so, I really don’t know how fast will this processing move.. Anyway, Vermont will accept application eventhough, you are not yet here in US..( Application for those who have passed the NCLEX exam, but do not yet hold a license in a US state can apply ). Go to their website at www.vtprofessionals.org and you can find NCLEX RN passer endorsement application. So, I would suggest you have to do it fast especially to those who have all the requirements and to those who have NCLEX result soon to expire.I hope I have something to share to my fellow nurses.

Good day! Please help me. I dont know where to seek help. My name is Rhea Sandoval. I graduated from a new nursing school. I passed the local board last June, 2008. I applied for nclex (California) after graduation. After 3 months of waiting, they sent me mail requesting my school to submit basic nursing program. I sent it to my school together with the enclosed sample. The dean of our school gave to me the envelope and I was the one who mailed it to California.

After 1 month, a letter from California arrived stating the same problem: detailed course description of the whole nursing program(detailed course description). I immediately sent it again to our dean, but its been 2 months already and still they are not doing it. I’ve been trying to contact her but shes always out and shes evading my phone calls.
Okay here is the thing maybe your dean is not the best person to approach with this requirement. Have you tried contacting the school registrar? Because usually they are the ones who handle these papers. The reason that the CA BON is asking you for this is because you guys came from a new school and maybe they do not have records coming from your school before. I personally, had the same experience a few years back when my school changed its status to a university. They gave you a sample of how it should look like and basically what it should state are all the subjects that are being taken during the entire nursing course and a description of each subject plus the number of units etc. I hope this bit of information helped and let me know if you have anymore problems.

No problem, thank you very much for supporting the site. I did take the NCSBN review it was okay. I mean if you are the type of person who have access online 24/7 then this is a good review. Because you really get to do it on your own pace. But don’t expect it to be lengthy in terms of explanation because as far as I can recall its really summarized. Basically, when you get in to this review it expects that you already have a strong foundation on nursing concepts which is what most reviews really are.   • Request for transcript form
  • Breakdown of educational program for international nursing programs, (and lastly)
  • Nursing program verification form

Yes all these forms will be submitted to the school.

Yes that would be fine even without the exact date.

and correct me if im wrong. ung mga subjects na nklagay sa form (anatomy, microbio, ms, mga gnun, tpos my 3 empty blanks sa tabi), ung school na mismo mg fifill up nun dba?

Like mentioned above the school would be responsible to fill all these information out.

last, ung application form po. under dun sa PROFESSIONAL EDUCATION, dun sa “CA nongraduates”, anu po ung ‘date nursng requirements completed’? un po ba ung graduation?

Yes that is your graduation date.

-mgsstart na po ko mgpass ng application dito sa CA, uhmm panu po un? pwde ko po ba ipasa na ung application form? khit wla pa ung tor (and the other documents from my school) ko? pinaprocess pa lang kc un sa pinas eh. pwde ko na ba iuna ung application? or much better na hntayin ko ung tor tpos sabay ko nalng ipasa lahat2?

You can send your completed application form, fingerprint card and fees to the CA BON already. There is no point of waiting for the school documents since it would be coming from the school anyways. Just make sure to follow up the school documents or else your application will not be processed without it.

-and sa anong envelope po ba dapat ilagay ung application form? ung brown one ba? if so, wat size po?

Yes a short brown envelope would be fine. If ever you accomplished a fingerprint card just make sure that it won’t be damaged or bent.

-and same goes with the forms na ipapadala ng skul sa CA BON. anung envelope po ba? dba po kasama din ung mga request form ko na binigay sa kanila? so basically issend nman nla ung mga request forms ko sa CA BON, along with the actual forms. (TOR, etc) and pwde po ba itiklop ung mga un?

Yes it is fine if they all fold it in one mailing envelope. To be sure have them sign the flap of the sealed envelope for security reasons.

Thanks ms joan!! may question pa po ako, is there any other way para hndi na ako magretake?? like endorsement sa ibang state na hindi nagrerequire ng SSN to issue permanent license?? pwede po kaya yun??

That is what most people do they endorse to another state. But that of course includes more expenses since you have to fulfill the requirements the state you are endorsing to.

I am a filipina and I am currently residing in US now. I just would like to know if it’s possible that I can take the NCLEX-RN in California even if I have not taken the Licensure examination from my country of origin.

This would be a case to case basis. Most of the people I encountered who are living there but studied in the Philippines had problems in applying. But what they did was they explained their case with their assigned evaluator. I guess what you can do is submit the necessary requirements and then if asked why you do not hold a license in the country where you were educated you state your case.

Hi po Ms. Joan newly grad. po ako ng nursing sa philippines at nandito na po ako ngaun sa california. Bale immigrant po ako at bumalik lang ako sa philippines para mag-aral dun. Right after graduation po bumalik na ko dito sa US at dito ko na po pinagplanuhan na i-take ung NCLEX. Sinend ko po sa knila ung mga alam kong requirements for the application, last week po i received a letter from Board of registered nursing na kulang po ung nai-submit ko sa kanila.Ito po ung:

  1. Written explanation of not holding a license
  2. Transcript(both theory and clinical hours)
  3. Educational Breakdown form

Dun po sa no.1, may nakita po ako sample sa inyo na pwede ko pong magamit. Sa no. 2 naman po eh ang mali ko ako po mismo yung nagpadala directly to the board na dapat ung school pala tapus po ung sa no. 3 eh pinadala ko na po kasi sa knila ung breakdown pero bakit po hinihingi pa rin nila sakin un..kelangan po bang ung school din ang magpadala nun kasama nung transcript ko?

Ang tagal ko na rin po kasi naghihintay dito wala naman po ako idea kung panu talaga ang tama kong gawin. Pwede po kaya i-fax na lng ng school sa BON ung requirements ko? Ang tagal po kasi sa mail.

The TOR and Breakdown form should come from your school. They should be the one mailing it. Unfortunately, the BON does not accept fax it should be via mail. You should inquire from your school if they offer special courier service such as FedEx or DHL. The best thing to do to get this done is ask someone from the Philippines (a relative/ friend) to help you in submitting the necessary forms that the school needs to fill out. And then, if your school does not offer special courier service what you can do is have it claimed by your representative just make sure all the documents are in a sealed envelope with the registrar’s signature and then let your representative mail the documents. But please take note when mailing the documents to use the school’s name and address as the senders address not yours.

The answer is yes. But this only applies with regards to the SSN. But once you get your permanent license there is no need to retake the said exam.

hi. im just a new resident here in CA.  im a registered nurse in the phils. i want to start my application for nclex here in CA. But i think maybe that’ll not be possible right now. I have a little problem about the transcript of records (TOR). As written on the rn.ca.gov, theyill not accept trancript form that is “issued to student”.  Well correct if im wrong because the way i see it, you still have to mail or contact your school, then request a TOR and mail it here in CA.  Is that right? Heres my question:

  1. Where can i get the “request for transcript (TOR) form”?
  2. Assuming that i have my request for TOR form, at what address should my nursing school send it? will it be my address, or will it be directly to the BON?
  3. Can you pls kindly explain to me what is “Breakdown of Program for International Nursing Program”? Do i need to get this one also?
All school forms such as the transcript, breakdown of program for international nursing program and nursing verification form should be filled out and sent by the school. The forms are included when you download the application form at CA BON. The school should send it at this address if you are having it sent via special courier (FedEx, DHL etc.):

But if they would just use regular (via Post Office) they could send it at this address:

The Breakdown form like mentioned above would be answered by the school basically it tells the number of hours per subject area. Also the nursing verification form would be answered by the school and it tells what kind of teaching methods were used in the nursing program.
All the forms that you need would be available at the CA BON site.

hi ms. joan..i failed my NCLEX exam last October..ask ko lang po kung wat are the steps in reaaplying for the BON-CA exam? do i have to have a bankdraft again?or have my documents verified? thank you so much
I am very sorry to hear that. Usually when you get your results, there will be an application form for reexamination that comes with it. All you have to do is fill that out plus pay the application fee (bank draft). And then wait for another Letter of Eligibility (LOE) to arrive and then register again at PearsonVue.
I know this must be very hard news to take. Just take time to recover from this and then assess yourself on what you think went wrong during the process. When everything is settled, start fresh and prepare for the exam again. Good luck!

Hi coded mushroom, i have questions po regarding nclex RN ng California nagtake po ako nung dec at sinwerte naman pumasa, hinihingian na ako ng SSN and I have 3 years daw or my file will be destroyed.  Now I understand this means I have to be in the US by then, question is how, what should I do next and ano ang pinakasure na paraan? kung aabutan ako ng 3 year deadline what will happen? what can/should I do? please help me
Hi Roan! Well first of all thank you for following my articles at CodedMushroom. As you can see I have my own site now dedicated to people like you who are having problems with regards to NCLEX applications and other stuff. Now with regards to your question, its pretty tough to be in that position since you have no control with regards to the retrogression. What other people do is they transfer their license to other states that way it gives you a fresh start. But then again, doing such involves time and more over money sine you would have to pay application fees and fulfill other state requirements.

hi. i already received the fingerprint card from BON of Cali. Too bad that it was a bit folded when i received it. Since i have read that folded cards would delay the processing of my papers, would they send me another cards again? Hoping for your reply

As long as the damage to the card is not major it would be fine. But if dents and folds are too visible that might be a problem. They will send you another set of cards but you would have to request for it again at their website. You can request for a copy of fingerprint cards at the link provided.

hi, pano pala pag pasmado ang kamay pag nagpapafingerprint for the nclex application?

Usually when you go to NBI branches there will be somebody there to assist you with the fingerprinting process. These people are professionally trained to get your fingerprint marks that is why it is adviced to seek a professional when obtaining your fingerprint.

First, which state did you apply in? Usually, when you get your result they send you an application for re-examination. Like in California, they just ask you to fill out the re-examination form and pay the fee. Then after that you wait for your letter of eligibility to be able to take the NCLEX-RN exam again. Also, please be reminded that PearsonVue has a time period before you can retake the NCLEX-RN depending on the state you applied in. Usually they allow you to take the exam 45- 90 days after your first take. And what do you mean change of address?

i am an RN license in NY, i’ve been working in NY but since there’s retrogression, i couldnt renew my employment authorization and my NY RN license is about to expire too. will i be able to renew my RN license without greencard or Employment authorization?

I am not quite familiar with your situation. But how were you able to obtain work in NY in the first place? Nonetheless, NY Board of Registered Nursing has an online renewal form for professionals you can view the link here and see if you qualify.

i already received the eligibility to take the NCLEX-RN exam. but i am still not calling them or registering with the testing vendor Pearson VUE. my question is, how long will they give me to review before taking the exam? are they going to give me enough time to study?

Usually, when you reigster with PearsonVue your Authorization To Test (ATT) will be valid for one (1) year. Please take note that aside from registering with Pearson Vue you would also have to schedule the exam date in which you would want to take the NCLEX-RN exam.
